Application Submission:Submission of an Enrollment Application does not guarantee that your child will be enrolled. Your child will not be considered for an enrollment vacancy unless all required materials are submitted with the application. Families must meet the income eligibility requirements.
The following materials are required to process your child's enrollment application. Please be sure to bring them with you to your enrollment appointment.•Your child's Birth Certificate. •Your child's immunization record (obtain from your Primary Care Provider).•Proof of any Public Assistance that you may be receiving (TANF or SSI only).•Income for the past Tax Year (1040, W-2,) or current year pay stubs.
